2. FAX forms to PDS at 800-399-5423
3. Receive Chain of Custody form that an applicant takes to a collection site, and work with your customer service representative on sites convenient for your workforce.

How payment is made:

PDS can arrange payments in one of three ways:

1. Billing upon approval of a credit application;
2. Billing without a credit application as long as the account is secured by a credit card.
3. Provide PDS with a credit card for monthly billing. (Visa or MC)
